Continue to get a login page when I run my crystal report

I have a simple report using a stored proc as the datasource. I added the CrystalReportViewer to a form, and the ReportDocumentComponent to the form. No code at all.
When I run the report, a login form opens up with no information in the pull downs for server and database.
I get a login failed message if I continue...

I don't understand why this is happening. What am I missing here.... Thanks
