I need to produce a report based on the 3 field: ID, Date, Activity. It need it to look like the example below. I have worked for hours to no avail. I have tried Pivot tables, which return the ID and the date information OKAY; however, I don't need totals. I have tried Crosstab query, however, I don't need a "value".

Line ID SJU SJU SJU
MIA MIA MIA

Use a crosstab. Put your ID as the row header, Date as the column header, and the First([Activity]) as your value. If you don't have multiple entries for each ID/Date combination, your data should be fine that way. --
