Change Password Http page on OWA, page not found error

I need help. I have configured the password change option on Exchange 2003 Ent/Windows 2000 Advanced, as described in the following link:

But when I click the Change Password Option I get an error that says "the page cannot be found" Http error 404. Does anyone have any idea on what I am missing?
