Cant' be heard

I have a branch that has IP sets registering back to a S8730 server running the following code:
Operating system: Linux 2.6.18-53.AV17PAE i686 athlon
Built: Mar 31 22:21 2009

Contains: 02.0.947.3
CM Reports as: R015x.02.0.947.3
CM Release String: S8730-015-02.0.947.3

UPDATES:

The end user has a 4621 ip set with fw 2.887, When she gets a call on her first line appearance and someone in her pickup group answers it and they put the call on hold she goes to pick up her line 1 and she can't hear the person. I have been looking through the PCN's but have not found anything that describes this issue...anyone else had this happen. We have 3000 ip sets and this is our only complaint, but we have swapped out her set and it still happens, we can duplicate it too.
 
doesn't this have to do with that whole data or line privacy setting?
