Calls Dropped with 3.0

I have not seen anyone else with this issue so I think it is more configuration than a bug in 3.0.

I have an SCN with a 403 with 1 30 Analog and 1 30 Digital expansion module. The other site with a 406 and 2 30 analog and 1 30 digital expansion modules and centralized VM pro. On memorial day I upgraded from 1.4 to 3.0.
Since then I have had the same issue at both locations. Calls will ring once and appear and then disconnect. This can happen on incoming calls from a PRI to a digital extension via DID, or an IP extention on the transfer from autoattendant. Most nototrious is between digital and analog extension on the same switch. The users ended up having to redial until the other end picks up on the first ring.
The calls will stop ringing and go to voicemail if it is on or, if a transfer, to hold or, for extensions without voicemail, just disconnect.
Sometimes it will occur really consistent on a particular extension but then the next day not happen at all, other times the first try will fail but the second won't.

Never had the problem with 1.4 and it happens on multiple extensions and types of extensions across both sites so I think it is something in the configuration. I didn't do anything with the config when I upgraded, it added the appearance buttons automatically. I have ran them through the wizard and I have not found anything that looks like a problem. I am wondering if it could have something to do with skipping a version or something. Should I try creating a new config file or something?
I am not sure what I should look for in Monitor.

This is my second time detailing this issue and I have not gotten any response so any advice would be very appreciated.

Did you upgrade thru all of the major versions in order (including 1.99 and 2.99 since this is a 403)?

Creating a new config from scratch is always the best thing to do. However, this can take some time to complete.
Using the Wizard will speed up the work ( and you can build the config offline in your office ).
 
Yes I upgraded the 403 properly through the steps, it is operating fine.

I will try creating a config from scratch with the wizard and see what this does for my issues.
