Call Drop using Forwarding on SIP Trunk

When a call is forwarded to a mobile number, the mobile phone rings. Once you pick up the mobile phone, the call drops immediately. You don't even hear any silence. This is also happening on twinning calls. The system is IP Office 500V2, 8.0 with SIP trunk on LAN 2, Open Internet, Locked Codec G711, RTP Keep Alive Enabled. Outbound and inbound calls all working with no issues. All outbound calls using the same ARS via the same SIP trunk group. Any thoughts as to why this is happening?
 
I have a idea : something is wrong. What is wrong? Can't say, can be anything from bad programming to failing hardware.
A good start is to monitor what is happening with the monitor software, default filter and SIP filter enabled (turn HEX options off).
Then study the output and maybe you'll see the problem, if not post the log file here but do not forget to tell which ddi number is dialled and to what number it is forwarded to otherwise we will be staring into the black evening sky wondering why we can't see anything
