Call Connected to Agent but still hear announcement?

Has anyone come across this problem before??
We had an instance yesterday where a call was put through to an agent and they could both hear one of our announcements.

Does this relate to any hardware/software problems that anyone is aware of.

Or is this something that will just happen like phanton calls.

Have you updated your firmwares. What version of software and which switch do you have ?

We have a Definity G3R V9.
We have support - is updating the firmware something they should do?

Maybe they're hearing an incorrect whisper page?
