Broken NCOS

About a month ago, we changed a dedicalted local only out bound T-1 to a second tie on an Option 11c 25.40b to our CS1000M M/G 5.0. The programing was never very good on the outbound so it was hit or mis as to what went out it vs. the tie. Now i will get reports of some local calls that can't be dial so I change the RLI and its fine. Today i got a call about a phone that wasn't working and found that my NCOS isn't working right now. The phone in question should be local only but in order to make it work I had to give it an NCOS with long distance. Any ideads in whats really broken here?
