Birth date collector

Hello .. can I find a free cgi or perl script can do the following :

1- collect the birth date of my site visitor through a forum
2- store this visitor's information : name – birth date – mail inside a data base
3- send an email on the visitor's birth day .

That's fairly specific, I'd say you're looking at hiring somoeone, or have a go at coding it yourself...
