Backup Fails in BE 8.6

Why does our backup fail every now and again. It will run succesfully for a week and then the next week it fails.

it seems to backup 100% of the data but it says unknown percentage but the size matches around the size of a successful backup.

Any suggestions? It backs up 10GB of data using a 12/24 GB tape
Its a DELL 2400 poweredge
256mb ram
35gb of free disk space in a raid 5 config.

Is it a memory issue?

any help would be great.
