Auto Generate Database

I basically have a program with four text fields. When they have data in them and are submitted. I would like the values to create fields in a table in a new database and I would like the database to be saved in the same directory as the .exe file. One of the text boxes are labeled "TextBox1" and the database can me "db.mdb" the button is labeled "ButtonGo" Thanks any help will be greatly apresheated.
