Auto Dialer phone list

I have a form with several fields that each contain a unique phone number. What I would like to do is use auto dialer to have the computer call a number.

Any suggestions on how to have a list appear where the user can select the specific phone number to call by clicking it and then auto dialer dialing the number?? Any suggestions will be helpful. Thanks!
