Audio Hissing, lagish video out put

I have a problem with Premiere, when I try to output the audio or preview it in the monitor there is a hissing to the audio track and sometimes, the video lags a bit. What are the optimum file settings for a good TV production? And How do I stop the hissings? I know for a fact aht the audio file does n't have hissing because when I play it on a regular MP3 player, it's fine.

ANy ideas?
