All phones Dead

IP 500V2, Dig 8/PRI, Dig 8, Combo Card, R7. All 1408 phones are dead/no display at all. PRI still works as calls come in and go to voicemail (vmpro works fine). No lights on the cards showing connectivity, but heart-beat lights working. Logged in remotely and system recognizes all modules, but in Sys Status it shows all extensions as "unplugged". Tried a reboot, same issue. I have a feeling that something has been connected incorrectly and is shorting the system out, but wanted to see if anyone has had an issue like this before.

Also, the client did unplug each port one at a time and there was no change. I told them they should unplug everything, power cycle the system, and then start plugging in one at a time.
 
I would shut the IPO down and remove all cards and take a good look at them to see if there are any blown up ports on it.
Does SSA shows the cards in the system?

Then i would start with swapping the IPO and or cards to see which one is the problem.

The combo card was shot. Traced all connections to it and a new cubicle had the computer and phone reversed so the computer was plugged to the IPO combo card. Unfortunately, the card is blown. Had to replace it.
