ACD login problem

My customer has an SX-200ICP system.
Software version 4.0.1.20

It crashed on friday (hard drive failure) I got a new system overnighted and installed using a system backup file. Everything is up and working, except 1 ACD phone will not login. All of the other phones login. When I hit the login code the phone shows "no access". They are all 4025 supersets. The CLOS is the same as the other phones.

I know I ran into this problem once before, but I cannot remember the solution
 
I tried deleting the phone and put it back in and it still comes up "no access"

 
Did you watch the data restore on the maintenance port? Any error messages/failures?

Have you performed a DBMS Check Full?
 
Everything loaded properly, there weren't any errors. I have 8 ACD agent licenses only 3 phones are currently logged in, all of the other phones can log in. Just this one keeps coming up no access.

What does the message mean? That phone isn't allowed or all of the agents are in use?
