9616 & 9630 Locking up

What would cause a 9616 or 9630 to lock up? We are having an issue with ramdom phones locking up and the only way to reset them is unplug and replug to get them to work. The phones are on Extreme POE switches and are not fully populated. Is it power, cabling or TFTP issue?

I will check to see if it is up to date because there are approx. 100 phones on the system and it is happening to a small group. What is the latests firmware?
