6408D+ goes blank in the middle of a call?

I have an agent who is on a call then the call drops and the phone goes blank ( phone displays blanks out then comes back on) Could this be a hardware error or something else?

Sounds like either a faulty phone or line side power issue if they don't have a power supply box. Replace cable, phone, punch down block....

I know this is very basic but sometimes the most over looked.

If it is a IP phone, I would make sure that it and the related media card has all the latest updates. Otherwise I would suspect a bad phone or intermittant wiring problem. Does this agent have a habit of standing up while talking and pulling on the phone?
Is someone else with the same type of phone having the same problem? Sometimes a nearby fax machine or other electronic device will induce problems into a line.
