5621SW reloading bins constantly

Hi,
IPO 403, 3.1.65 release. Installed eight 5621's over a VPN link, all came up initially, now one of them constantly downloads the same bin file ( b20d01b2_3.bin) over and over again, even after reboots and unplugging the phone. Is the phone bad or is the firmware corrupted, is there a software reset on the phone? Also, I have uninstalled and reinstalled manager. I would call our tech support but I don't have the time to sit on hold at this point, and the issue isn't pressing yet as the user won't be at the desk for a week. Don't know if this will help but here are some excerpts from the TFTP and SYSMON logs - they seem to repeat the same sequence over and over again.
