I have a site with 10 5621SW IP phones, a VCM 16, a 406V2 at 4.0.14, with embedded VM.

Last monday and this monday, all of the phones go to Discover. All stations are connected via a Netgear 24 Port switch except one, which is directly connected to the IPO. Upon failure we are unable to see the IPO from the Admin machine, which is also uplinked directly to the IPO. The only way to bring the phones back is to power cylce the IPO. Any thoughts?
 
We have seen the LAN ports on 406's lock up several times,this causes the issues you are getting and the only way to get them back up is a reboot. If it is happening more and more the unit could be on it's way out.

Did you step the upgrade through 3.1.999 loader?
