3500XL "output buffer failures counter" meaning ?

What's the meaning of the "output buffer failures" counter in the sh interface fastethernet 0/x command. I can't find anythinf in the doc.
On some ports i have some for example on a port wich is connected (full, 100 Mbps) on another switch.

Also, some ports (on other switches) blink orange, and i see nothing on the show interface or sh controllers, but only up and down lines in the sh log (which is difficult to see). When i connect a test tool in the plug i see taht the link signal goes up and down. Can i trap this in the cisco stats (or Is there a tool to fetch lines which are very close in time in the show log related to the same port...).
