1608 and 1616 hold & transfer trouble

Just got news from customer they brand new 16xx. Series phones they received have trouble with th hold and transfer button. Sometimes they just do not work.
Some sets just die after unplugging.

Any one seen this?

The difference with olders sets is..... These say Made in China

Please let me know if the information that was provided is helpfull.
