1408 phone losing firmware?

I have an IPO 8.1 (73) with embedded mail. We are trying to add a new 1408 phone. At first the phone was just showing a= on the screen. Otherwise unresponsive. Manager/System Status showed the port as unplugged.

I had the tech plug the phone directly into the port on the Combo card. I changed the TFTP and HTTP server IP addresses to the PC that has Manager on it, rebooted the IP Office and kept Manager open during the reboot. The phone came up properly and I could see it in System Status.

Sent the tech back onsite to move the phone back to the intended location. He saw that the phone was displaying the correct extension number. He unplugged it from the Combo card, moved the patch cable into the port, plugged the phone in at the jack, and now we're back to a=. He took the phone back to the combo card and still the same: a=

Any suggestions about what's going on??
 
No point the http and tftp to the manager address, and redeploy phone.
 
redeploy the phone" - how? more than just unplugging it, because that didn't work. Is there a mute+RESET or CLEAR command like on IP phones?
 
If the port on the Combo card already updated a phone then it will not update another phone plugged into it until you reboot the system. Have you rebooted the system? And is your phone file server type set to Memory Card (assuming this is a V2 chasis)?
 
I did reboot the system, the file server is set to memory card. When I did the reboot I kept manager open. It didn't work.

I'll try again.

I am concerned about why it lost the firmware in the first place...if it works again now, will it go bad when the tech moves it again??
 
Are you sure if the phone is attached to a proper port, your description gives me a clue it is connected to a analog station port and not a digital station port.
If it is a digital station port for sure then check wiring by plugging a known working 14xx phone into the jack.
On a combo cards ports 1 - 6 are digital station ports, ports 7 & 8 are analog station ports and depending on the type of trunks:
if analog trunks port 9 - 12 are trunk ports, in case of BRI ports 9 & 10 are trunk ports and port 11 & 12 are non-functional.
If it was a fileserver problem the phone would say "Please wait" instead of a "a=
 
Actually the phone is connected to a Digital Station 8, not a combo card. It's plugged into the 3rd port (2nd from the top left). I had it working there once!

Did another reboot last night and the phone is still showing "unplugged."

I have seen some suggestions of using the IP Address of the IPO as the TFTP and HTTP.? Will try that tonight unless another suggestion comes along.

 
I wanted to update everybody and thank you for your suggestions.

The phone is working. The port is working. The TFTP and HTTP server IP addresses are set to the PC that has Manager on it.

We tried different ports and different phones and about three reboots later, all of a sudden the phone came up - in the original port. I didn't question it, was just glad to see it!
