You need to either use bridged mode on the modems or have a device to terminate PPPoE. I'm not familiar with that DSLAM to know if it can do the termination itself but that would be very unusual.

Do you see any errors/messages on your PPPoE concentrator? If it's a Cisco can you paste the relevant sections?

LCP is part of the PPP process, are you using PPPoSomething or is this a bridged device?

Providing analog and digital at the same time shouldn't be a problem with a IAD2400 as long as they have enough slots. It should just be a matter of configuration.

Put more/better/different filters on. We have seen some cases with a fax/modem switch that needed 6 filters cascaded to make the DSL and fax work consistently.

Don't bother with the software, they aren't that difficult to program with a phone if you have the docs. Some of the things on them are irritating, such as how you denote a line that has a card but no line attached. I was given one in a box and a 3" binder one morning and had it going by lunch time.

The IP Office is different - think of it as more emulating a PRI over SIP. It doesn't work well with providers where you have to register each number - emulating POTS over SIP.

Cell scrambling is generally only valid on an ATM circuit. Are you using an ATM port adapter or a HDLC port adapter?
