DoCmd.OpenForm always opens a new form, not a subform on a main form. I’m not familiar with “navigation tab subform”. Is there actually a tab control on your form?
Could you share your table structures? Do you have a table with multiple true/false fields for checks? If so, this would be “committing spreadsheet”.
We need more clarifications. Typically records aren’t deleted, they are simply filtered out of forms and reports.
Office 2000 is about 25 years old. Have you considered upgrading? If you (or someone else) opens the file in a later version does the problem still exist?

Have you attempted to troubleshoot by writing similar code that sets the picture of a single command button with hard coded values? Isolating bit by bit is a great troubleshooting method. Don't add any complexity until you have success with the previous step.
Can you give us some information regarding your table(s)? Are you updating a single long text field in a single record or adding a record with each note?

Any other databases with the same errors? There could be a corruption in the Access database possibly within a table.

Your settings match mine so I doubt that is the issue. It may be the Windows update. I would try to find a PC without the update to test. The other possible solution is to roll back the update.
