Yes. Go to the settings you mentioned on primary server and chose "create certificate for a different machine". Ensure to fill the SANs correctly and download the certificate as p12 file. The p12 can then be applied to the secondary server
You don't want the AppServer to be placed directly in the internet. You sound build a VPN or another private connection between Azure and the local site and let the AppServer have a private IP.
I like the J phones. Especially the Webinterface for troubleshooting.
The main disadvantage is the long time to boot and the fact that even after a reboot the phone doesn't update its cinch with all changes made in 46xxsettings.txt. This is only done when the phone is reset.
