Also, fa0/1 seems to have some input errors which may or may not be recent, again check speed and duplex mismatch.

I would ensure the 3560 switch is got a clean config and also ensure the vlan config is cleared before connecting to your network.
I would use vtp transparent mode, then create 4 new vlans which are only available on this switch, ensuring no layer 3 interfaces are created for the 4 vlans...

Please post a scrubbed off config of the switch.
Also the log does not indicated it is a problem with STP.
I would start by taking off the port security and see what happens.
If all works okay, then I would do a show mac address-table int gi4/0/34 dyn and see what comes up.

You can do a continuous ping from your host and do a show int fa0/1 and check that the your output packet value increases, that will be an indication that the traffice is getting to the router.
