    you didn't need to add the new server account to the DC before joining the domain try deleting the account and the dns record and then joining the domain again. make sure that when you delete the account that it replicates to the other DC You will also need to use a Domain administrator account...

    if you are not getting the create mailbox option when you create new users in active directory you can actually install the adc on the domain controller. I have also found that this fixes the mailbox problem.

    you dont need to install active directory on the exchange server just join the server to the domain and then when you run exchange setup it should detect the active directory. If for some reason it does not detect the domain controller you may have to install the active directory connector...
