other things to check are that the scsi id's are set with the autoloader at the lowest id and the drive a higher id. verify that the scsi cables are correctly orientated. Host to AUTOLOADER then to the drive, termination at the drive. Make sure you are using DLT IV media...

go into the Scsi card bios at power up and check that the card is set for wide negotiation and the transfer speeds are at the highest for all scsi id's. that is a good place to start
it sounds like the drive inside the autoloader needs replaced or repaired. The please clean me usually means that the drive cannot read or write data to the tape. It will be able to rewind and go forward without reading or writing. Hope this helps
power cycle the library. when the display indicates download BTO.x. press the execute button several times. it will after a few seconds bring you to a config screen where you can select the number of drives in the library.

it sounds more likely to be the way you have your jobs set up in legato. Is the media set to overwrite the data or append. you could be just adding data to the tape all this time, and not realising you weren't overwriting the old data until the tape became full.

my advice would be to enable autoclean. None of the manufacturers of LTO really give an indication of how often to clean the drive, They advise to clean when the drive requests to be cleaned. Cleaning tapes are abrasive and wear head life so I would clean as little as is necessary.
