I have found the cause of the problem, all extensions were in the ICDG, but only 1 had Immediate ringing, all the others had "No Ring" set, which proves the theory of the maintainer change. I have now set the extensions required to "Immediate" and all works perfectly. I have also changed...

If you put ! as a translation pattern, then put the destination you want unrecognised calls to go to in "Called Party Transform Mask", then all unrecognised calls will go there.
