Bad Phone.
Test the 3904 at the TN and Test another Known Good 3904 at the TN. If the good phone works without the problem it's the Phone.
You could try to Flash the Firmware on the Bad Unit and see if the problem clears.

What Type of Phone, 2008, 2616, 3900?
Sometimes you have to have a Feature between the Phone with the XMWK and other Lines. The XMWK is generally a Lamp Feature and has nothing to do with the Lamp.

ACD Phones are built in LD 11 and or Load 20. Nortel suggest Not to Copy or do Easy Changes on ACD Phones. Even if you do a single change on an ACD Phone, you Out the Phone and rebuild it.
