I've verified that, for my site, it is InoculateIT real-time a/v scanning that is causing the problem. The error has not happened ONCE since I've turned it off. Unfortunatley, that has the side-effect of leaving the system open to viruses!

If you're going through what I went through, it's real-time ant-virus software that's causing the problem. I've disabled the r/t scans on my Exchange server and the problem went away.
I've had to do a lot of trial-and-error research on this one myself. It seems to be A/V software related, though. We're using InnoculateIT A/V set with full scans nightly. Since I've changed the scan schedule to Sunday only, the hangup seems to happen a LOT less now. If anyone's using...
