With the push to migrate things to the cloud, there's obviously still a few things you can't pull off like you can on good ol' on-prem hardware, leaving a deficiency that I'm hoping can be filled...
Phoneman2 wrote an awesome FAQ back in 2003 that showed people how to write a vector so the system will speak back to you with the extension you're calling from (great for analog ports, and older digital phones that didn't natively display your number on the screen)...
FYI, several Avaya certs recently expired for existing firmware, meaning they had to be upgraded prior to the expiration date. I believe IPSI's were on that list. May want to check with your vendor to see if that's whats causing the issue.
I have (both speech-enabled or dtmf). But it wasn't anything odd - not like I did anything special, it just worked.
I am a little bummed that exchange doesn't support DTMF transmission via INFO packets... :(

Yes, this is all SIP. Both inbound and outbound. Those SIP adapters/converters sound like crap. We used to deploy the Audiocodes 118 and it was OK, but always sounded like an analog line (unclear).
This is all-SIP.

Are these IP phones on each site with Direct Media enabled, or TDM stations? If IP phones, try disabling Direct Media for a little while, see if the problem goes away. I know, you may not have enough VCM resources for that, but at least try at one site. I've had wierd buggy experiences with...
Its supported on the system in 5.0. You can flag the trunk to support PAssertID, which allows it to send the actual SIP info and the "show THIS number instead" flag with different data. It will pick up your Nsi5132411010 string (or whatever number you use) for the asserted ID.
