I have four (near) identical forms. I have four (near) identical queries.
The forms each have a (near) identical Listbox, which populates based on their respective queries.
Two out of the four forms work perfectly. The other two, the listbox is blank.

Yes, it would be located in the Address book. If you don't have a Domino Administrator client, open up the NAB. Expand Servers, and there should be a section labeled "domains".
In there, you'll see it if you have one.

Are you connecting/sending mail through a Domino server?
If so, do you have a Global Domain document set up?
In the Blobal Domain document for your company, switch to the Conversions tab.
There is a field there that says Address Format. You can change this to "name and address"...

I've found others that have had this problem, but found no solution yet!

As long as the users have permission to access each server they want to, it's just a matter of creating Connection Documents in each client's address book to the proper servers.

Go to the Agents section of the mail file, and double click the OutOfOffice agent. Click on "Options" and there you will see what server the agent is set to run on. Most likely it's set to the correct server.
I have a feeling the problem is that users do not have rights in the Server...
You can put it wherever you want it on the new computer.
What you do is open Outlook, and choose File->Open->Personal Folders File (.pst).
You can then browse to wherever you saved your folder, and there it will be!
