Sorry I didn't update this with the final solution.
The problem was that there were about 30 "virtual" users on the Primary system (auto attendant forwards, user forwarding, mailbox only users) that were tying up IP Endpoints because 'auto-create extn' was left on by the original installation...

I am hesitant to delete the virtual licenses, as I suspect they are currently providing 100-percent of my current working phone functionality!
