You could use live Trace In Illustrator Cs1 and Cs2. An old School way is to cut a clipping path around the type and export paths to Illustrator in Photoshop.

For the font problems check to make sure your fonts have been embedded when you make the pdf, check your distiller settings, I know Truetype fonts can be a headache. If you have Pitstop you can embedd these fonts using this software. For the images I would try what genomon suggested...
You would need jmgalvin said. You would have to try and match the font or fonts that are in the scan or use copy and paste. If there are alot of pages it could take some time.

Is it possible that when you hightlight the text - check your trap setting - its possible that this type is set to overprint and not knockout. I see sometime on files that we receive.
Go to window > Show Trap Information.
Highlight the text what is the text trap set to in the Trap Information...
Can you save the files out as eps file? Then open the files in Illustrator then try saving the file out of Illustrator then exporting the file as a dwg or dwx file.

For word document I will create .prn file or ps file and I change my setting to Download as Soft font instead of Substitue with Device font- this sometimes helps out with Bullets - then I will distill my prn file.

Did anyone mention putting .qxd or .qxp at the end of the Quark file's name and the opening Quark and then Try File>Open and then open the file

Try creating a ps file, I think word creates a .prn file and try distilling this file instead of using the option in word.

The image are you saving it out of Photoshop as a eps. In the eps options window when you saved it as a .eps file - did you have any of the option checked such as "Include Transfer Function." I leave all these options unchecked.
I know if we rip a page and the image turn negative - one of these...
I would try High for a transparency Flattner setting.
I have my own custom settings:
Raster/Vector Balance: 100
Line Art and Text Resoltion 2400 ppi
Gradient and Mesh Resolution 400 ppi
Convert all text to outlines: On
Convert all strokes to outlines: On
Clip Complex regions: off
