My two cents...
If you have your steps recorded in an action in Photoshop, you might be able to work your way through them in Illustrator (you'll need to get creative...you can't simply import actions from Photoshop to Illustrator as far as I can tell)...you'll find some tools are not...
It was the EPS file. Talk about frustrating. The thing was hiding out in the furthest corner of the pasteboard! As soon as I deleted it, the swatch went away.

Here's a fun question...does this ever cause printing problems with commercial printers?
When I export the "mismatched" PANTONES to PDF, they still look mismatched.
