It's an I2004 Rel 09, MFG Date 040205.

Can I convert that to a SIP phone, if so, how?

It appears you can on certain 2004's in another recent thread but you would need to have a TFTP server and the SIP firmware for that phone.
Your best off to get a more up to date phone.

Carrier supplying a SIP trunk service or for a DNon a PBX?

Yes I read that post, wasn't sure if I have the same phone or not. Don't know what Phase 0 or 1 means.

I have some 2004 and some 2002 that I would like to use on another SIP pbx.
