Hi I have started creating a suvrey database.  I found a database created by Hook'D which has been very helpful in helping me in setting up my survey database.  The problem I am having is the set up of the form for entering the suvery responses.  I have listed all of my questions in a table and the answers in another as a value list for each series of answers.  I'm unable to set up a form with the question and answer undernth.  I think I missed something. Any help would be greatly apprecated.

Hi Lav,

Is it the case that a form will display a question, and needs to offer a selection of possible answers?

If so, then you're main form would contain the questions.
On the main form there would be a combo box - containing the list of possible answers for that question.

If that doesn't help, then let's go more detailed - ask away.

I'm not sure why you wouldn't use the sample form in "At Your Survey". It lists all questions and has a combo box for selecting answers.

I've since important the form into my database and am using it.  I didn't think I should use your form outright without permission.  I was just looking at it to get an idea on how one work.  I think your samples are great.  Thank you for your assitance.

Feel free to use the survey application for your own benefit. You can't "sell" my work without my permission.
